An Unexpected Error has Occurred


We're sorry, but an unexpected error has occurred.

We're not sure how this happened, but the support people have been notified of the problem.

Please note: The open enrollment application process is NOT a first-come, first-serve process. As long as you have your applications finished and submitted to DPI by the deadline your applications will be given the same consideration as every other applicant.


What to do next

If you were making changes to your information, those changes probably didn't "take", and you'll need to make the changes again.

If you were trying to log into the system, you may have used an incorrect ID and/or password. If this is your first time using the Open Enrollment Application system, you may need to set up a new ID and password by returning to the previous page and clicking the "Set Up New ID" button. If you previously used this application in a previous year, you may use that ID and password.

If you have forgotten your password, please return to the preceding page and click on the "Forgot your password" link, and you will be emailed your password. Please note that if you did not use your email address as your ID, you will not receive an email with your password.
